To all coaches, team parents and parents,
Medical release forms must be notorized. We will have 2-3 notories at the park on Monday the 18th and Thursday the 21st of August from 6:00-7:00 pm. If you cannot get these forms notorized during these times, you can contact a bank, an insurance company or check the phone book. The Notory must witness the signature being placed on the form. Coaches are then responsible for assuring that the forms are turned in at the Team Parent Meeting held on Wednesday the 27th of August at 6:30. The entire team must be turned in at the same time. Please write on the top of the form the team name and age group or the team code. WE MUST HAVE A FORM FOR EVERY PLAYER. IF A FORM IS NOT RECEIVED BY THE CLUB - THE CHILD CANNOT PLAY.
